2023 OIP Student Application Form

ALL STUDENTS TO NOTE:

Students going for OIP must complete the OIP application by 10 Nov 2023.

Please check your passport validity period and make sure your passport is valid for at least six months before departure. If your passport validity is less than six months, please renew your passport first before completing this application.

Your passport must be in good condition, with no torn or missing pages. If your passport appears to have been tampered with, you may have difficulties at a foreign airport, including being refused entry.

Photo Guidelines
- Photo must be taken must be clear, in sharp focus, not pixelated, and with even brightness, within the last 3 months
- Facial expression should be neutral with mouth closed
- Shoulders and hair must be fully visible
- Photo background must be white - Headgear/attire must be dark in color against a white background
- Spectacles must have clear and untinted lenses where the eyes are visible
- To wear non-colored contact lenses and ensure no red eye
- Eyes must be fully visible
- Facial features must be taken at full frontal view/Eyes must be looking at the camera
- Photo must not be distorted/flipped
- Digital photo must be original without alterations using computer software/mobile app

Housing Deposit Refund

All OIP students must fill up this portion to receive their housing deposit refunds. Students will receive a full refund from the housing deposit if there is no damage to the housing after check-out. The housing deposit refund will be in Singapore dollars and will be credited to your bank account indicated below.

NOTE: Information is only accessible by DigiPen (Singapore) Accounting personnel. All detail will be deleted when the refund process is completed.
