Upload a clear scanned copy or photograph of your NRIC, National Identification Card, or Passport. Ensure that all information is clearly visible and legible.

Upload a recent passport-sized photograph taken within the last six (6) months. The photograph should clearly show your full face against a plain background. Do not use filters, beauty effects, AI enhancements, or edited images.

By selecting "Yes", you consent to DigiPen (Singapore) taking photographs and/or video recordings of you (or your child) during the workshop. These images and recordings may be used by DigiPen for educational, promotional, marketing, publicity, and communication purposes in print, digital, social media, or other media formats.
If you select "No", DigiPen will make reasonable efforts to exclude you (or your child) from photographs and videos used for these purposes. However, we cannot guarantee that incidental appearances in group photographs or recordings can always be avoided.
