FERPA Block Request Form

Under the protection of FERPA (Family Educational Rights and Privacy Act of 1974), DigiPen will not release any information pertaining to a student’s academic, financial, or account records to any family member or personnel who may request information about an individual’s record, unless permission to release this information is indicated by the student by completing the Proxy module in Colleague Self-Service.

FERPA regulations and the Proxy module cover Personally Identifiable Information (PII). Another type of information, which FERPA regulations allow us to share with 3rd parties (only when necessary). Directory information at DigiPen Institute of Technology is currently defined as a student’s name, primary telephone number, Institute email address, major, dates of attendance, full or part-time enrollment status, and previous educational institutions attended.

If you would like to block sharing of ALL Directory Information, you may do so by completing this form. Please note that a Full FERPA Block will block DigiPen from sharing ANY information about you, and will remove your name/information from any DigiPen publications, graduation information, events, etc., and will block us from discussing you and your information with any prospective schools, employers, or other 3rd parties.
